This form includes all of the information you should need.

You can fill it out on your phone or computer and email it back, or print it, fill it out by hand and send me a photo or scan of it, as long as its legible.

Securing the Booking Date

A signed agreement and a non-refundable $50 deposit are required to secure the booking date.

Location

I travel anywhere in San Luis Obispo County and South to Orcutt. In some cases, I may travel further. I am coming from Grover Beach, so I do not charge a travel fee for events located in Pismo Beach (including Shell Beach), Grover Beach, Arroyo Grande (within incorporated city limits), or Oceano. Fees for other locations vary depending on distance. Please contact me for details.

Insurance

Faces by Gina and Company is professionally insured by Great American Alliance Insurance Company. Certificates available upon request.

Requirements

Client will provide a covered, comfortable area with at least 6 feet surrounding, two chairs, and one 4ft + table.
